Permanent Reservation Contract for September 2008 – June 2009

As a permanent reservation member I will automatically be signed up for any function for which permanent reservations apply.  I will not need to confirm my reservation by phone beforehand.  Applicable charges for the function will be automatically billed to my club card. _______Initial

 In the event that permanent reservations do not apply for a function, I will need to make a reservation by phone.  I will also deliver a check to the permanent reservations point of contact listed on the advertisement page of the Ka Leo Magazine for that particular function.  _______Initial

 In the event that I cannot participate in a function for which permanent reservations apply, I will call the permanent reservations point of contact listed on the advertisement page of the Ka Leo Magazine for that particular function, and cancel my reservation before the deadline time and date. _______Initial

 Cancellation notification(s) made to any person other than the individual listed as point of contact for permanent reservations in the Ka Leo is/are not valid. ________Initial

 The deadline date and time for reservations or cancellations will be noted on the advertisement page in the Ka Leo Magazine for all functions.  If permanent reservations do apply and I do not call before the deadline to cancel, I will be charged the full price of that function. I give the Hickam Officers’ Club permission to bill my club card for that amount and do not hold HOSC responsible. ________Initial

 In the event that I move or just want to cancel this Permanent Reservation Contract for any reason, it is my responsibility to cancel this contract with the Reservation Chair person only. All applicable charges will be billed to my club card for any missed deadlines until I cancel this contract with the Reservation Chair person. ________Initial
